Great job on the script.
You might want to replace line 104 with the "sets" command. "assignCreatedShader" doesn't always load up right away when opening Maya.
An option to keep the original geometry would be great. I could see people using this to render wire frames.
Also it would be a good idea to have the window load without doing anything and have a button for applying the modifier. Then the user can use the script on multiple objects easily without having to make a shelf button or run the script again.
